1) Format wise it should be fine, especially for the history section. It just still needs all the necessary information and clear to read.

2) Hmm, that's a good question. "Same actor" icons could happen even in non-OC situations and you have the right to use whichever face best fits your character. But you do have freedom of choice, so if you're concerned you could contact the player to see if they wouldn't mind. Otherwise, there's no real rule against it on our part!
