1) Holy crap Bugs Bunny—

/ahem.

Most of the traditional Fourth Wall concerns are related to the characters and how the players feel about them being told that they are fictional characters. So in that regard, we just want to keep it at a comfortable level. For the most part, statements and observations like, "Wow you look really familiar" or "You seem like you could have came straight out of a comic/book/video game" are valid as the implication is easier to handle as opposed to be directly told that they are a fictional character. Another valid statement is, "In my world, you are a fictional character" since that only implies that the character or at least a version of a character may exist in another world. For a current example, Mickey Mouse. It is very possible for canons set in modern day Earth to recognize him as a cartoon character and "know" him and could say so. So long as they have the "at least in my world" caveat, it's easier to handle. But in all cases, it's always best to ask the player for additional comfort security.

In terms of how aware he is about mechanics and such, he would not. For all intents and purposes, the game should be considered as a real thing, so that's where the fourth wall line would stop for him. Now, of course he can still make those aside comments (and probably get weird looks, haha!) in regards to other things.

2) TF2 characters are a bit tricky. Unfortunately, while I know of them, I don't know them that well. I do know that they're appable in some other games. Truthfully I would have to see the application itself to see where headcanon is used before I could really make a full judgment call.
